Abdominal Transplant Clinical Pharmacist Practitioner
UNC Hospitals Department of Pharmacy has an immediate opening for a highly‑motivated pharmacist interested in joining our Abdominal Transplant Surgery Clinics. The successful Clinical Pharmacist Practitioner (CPP) will play an integral role in the management of patients cared for in the UNC Hospitals Transplant Surgery and Kidney Transplant Clinics. The Abdominal Transplant CPP will serve as an advanced practice provider on the interdisciplinary health‑care team and provide clinical support to physicians, nurses, and other ancillary staff. The role encompasses pre‑transplant selection through post‑transplant disease management, including immunosuppression and management of opportunistic infections and other comorbidities. Responsibilities
Build a collaborative clinical practice with physicians and providers, determine appropriate treatment plans, and provide longitudinal dosing and monitoring of regimens. Align daily practice with medication‑management goals, including medication‑use evaluations and reconciliations. Educate patients, families, and staff on treatment plans, lab monitoring, and guideline updates. Manage toxicities, adverse effects, and transitions of care between acute and ambulatory settings. Collaborate with the Shared Service Center (SSC) Pharmacy and UNC Retail Pharmacies to optimize medication‑access processes and serve as a liaison for clinical and medication‑assistance programs. Educate Pharmacy students, PGY1 and PGY2 residents, and clinic staff; serve as a standing committee member and lead scholarly activities and research projects. Provide instruction to physicians, nurses, residents, and students through precepting, lectures, in‑services, and CE sessions; develop and implement quality‑improvement initiatives. Education Requirements
Bachelor’s degree in Pharmacy. Licensure/Certification Requirements
Registered Pharmacist and licensed CPP in the state of North Carolina; credentialing and priviliging completed through the UNC Health System accrediting body as an APP prior to start date. Professional Experience Requirements
North Carolina registered Pharmacist: BS Pharmacy or Pharm‑D, BPS certification or geriatric certification or ASHP residency program plus 2 years of experience. North Carolina Registered Pharmacist: Pharm‑D, 3 years experience, 1 certificate program. North Carolina Registered Pharmacist: BS Pharmacy, 5 years experience, 2 certificate programs. Seniority level
