RN - Care Transition Ambulatory Nurse - Family Medicine Clinic
The RN-Care Transition is responsible for provision and management of patient care in a clinical area for specific age and patient populations. The RN-Care Transition delivers care to patients utilizing the nursing process of assessment, planning, intervention, implementation, and evaluation; and effectively interacts with patient, significant others, and other health team members while maintaining standards of professional nursing. RN-Care Transition is responsible for accurately and efficiently triaging the telephone calls that are received from patients, their caregiver, relatives and other members of the healthcare team. This involves prioritizing patients’ health problems according to their urgency, educating/advising patients, scheduling appointments and making safe, effective clinical decisions under the direction of the patients’ physician. Position Requirements
Degree from an accredited school of professional nursing, and one (1) year of experience. Preferred: BSN preferred. Pediatric experience is preferred. License, Certification And/Or Registration
Current Alabama RN license. Current CPR certification or must be obtained within 30 days of employment. Traits & Skills
Must be self-directed / self-motivated; must have good communication and interpersonal skills. Must be able to: perform a variety of duties often changing from one task to another of a different nature without loss of efficiency or composure; accept responsibility for the direction, control and planning of an one’s own work; work independently; recognize the rights and responsibilities of patient confidentiality; convey empathy and compassion to those experiencing pain, physical or emotional distress and/or grief; relate to others in a manner which creates a sense of teamwork and cooperation; communicate effectively with people from every socioeconomic, cultural and educational background; exhibit flexibility and cope effectively in an ever-changing, fast-paced healthcare environment; perform effectively when confronted with emergency, critical, unusual or dangerous situations; demonstrate the quality work ethic of doing the right thing the right way; and maintain a customer focus and strive to satisfy the customer's perceived needs.
