Therapist - ACT NM ALB: Under the direction of the Program Manager, the ACT Clinician is responsible for assessing, planning, developing, coordinating and providing treatment, rehabilitation, and therapeutic support services for program participants. Responsibilities

Clinical Service Provision:

Provides therapeutic clinical services to ACT program clients, including conducting comprehensive assessments of psychiatric history, mental status and diagnosis; physical health and dental health; use of drugs or alcohol; education and employment; social development and functioning; activities of daily living and family structure and relationships. Provides direct clinical services including individual supportive therapy and psychotherapy to participants in individual, group, and family formats. Assists individuals in accessing resources and education related to basic needs such as benefits, food/nutritional support, financial support/education, transportation, and stable housing. Demonstrates the ability to recognize and assess suicide potential and risk and provides appropriate responses. Supports individuals served and their families in crisis situations. Provides on-call crisis intervention as scheduled. Plans, structures, and prompts social and leisure-time activities on evenings, weekends, and holidays as assigned. Service Team Approach:

Participates as an active treatment team member serving individuals in the ACT program, including recognizing and identifying symptoms of mental illness and substance use; participating as a member of a multidisciplinary team; coordinating with internal and external providers regarding service provision. Outreach, Engagement, Follow-Up:

Provides comprehensive outreach, engagement, and follow-up to assigned individuals, including contact attempts over phone, in person, in home, etc.; engages and develops rapport with individuals with a variety of symptoms/presentations. Cultural Competence and Diversity:

Provides culturally appropriate services to diverse populations that are person-centered, culturally relevant, and non-judgmental, with an open and welcoming approach. Documentation:

Completes documentation that meets expected standards, including documenting all services within expected time frames; ensuring documentation meets service definitions, regulatory standards, agency expectations, and quality improvement goals; ensuring documentation is person-centered. Efficiency and Productivity:

Maintains excellent organizational skills, uses time management techniques effectively, uses engagement skills, and consistently meets established productivity standards. Staff Development Activities:

Participates in staff development to increase skills and professional development, including completing required trainings, identifying training opportunities, engaging in cross-training, and attending supervision sessions with application of supervisory feedback. Other:

Completes other administrative support duties and special projects as developed and assigned. Qualifications

Education and Experience: Bachelor’s degree in a human service-related field and a minimum of 1 year working with the target population preferred, or Master’s degree in a human service-related field. Licensure: Licensed clinician or working toward clinical licensure (LCSW, LMSW, LPCC, etc.). Experience: Dependent on education (see above). At least one year of experience working with the target population preferred. Other: Valid driver’s license and meets driving privileges guidelines established in HR Policy 12.6 “Vehicle Safety and Client Transportation.” Knowledge and Skills: Ability to manage time to meet deadlines. Ability to recognize and identify symptoms of mental illness and suicide risk and respond appropriately. Good verbal and written skills. Bilingual (English/Spanish) preferred but not required.
