General Purpose
The Speech‑Language Pathologist (SLP) evaluates and treats patients, communicates with families, physicians, and other health team members, and maintains documentation of services in the medical records. The SLP conducts in‑services and training for facility staff on an ongoing basis.
Responsibilities
Evaluate patients within twenty‑four (24) hours, or one business day of physician referral.
Develop an effective treatment plan and obtain approval for services from the referring physician.
Treat patients per the physician treatment plan.
Supervise Speech‑Language Pathologist Assistants in direct patient care and related activities, following state practice act.
Communicate with supervisor and other health team members regarding patient progress, problems, and plans.
Participate in Patient Care Conferences, Utilization Review meetings, and Rehabilitation Conferences as needed.
Participate in in‑service training programs for other staff in the facility.
Record evaluations, daily treatment notes, weekly progress notes, recertification documentation per Speech‑Language Pathology and the Board State Practice Act, and governmental and third‑party payer requirements.
Record treatment changes per policy and procedures.
Instruct patient’s families or nursing staff in maintenance programs and caregiver training in preparation for discharge from therapy services.
Participate in discharge planning.
Act as a clinical preceptor for affiliating CFY/RFP Speech‑Language Pathologists and student programs.
Comply with the Speech‑Language Pathology and Audiology Bureau State Practice Act and facility policy and procedures.
Maintain positive, effective, and appropriate relationships with patients/residents, families, staff, and professional colleagues.
Qualifications
Education: Master’s or Doctoral degree in SLP.
Licensure: Licensed as an SLP by the state in which services are provided, or meet alternative requirements if the state does not license SLPs (including documented clinical hours, national examination, and ongoing education).
Proficiency in computer skills such as using Casamba and Point Click Care.
Physical Demands The essential functions of this position require frequent standing, walking, reaching, pushing, pulling, speaking, and hearing, with occasional exertion of up to 100 pounds of force, and the ability to climb, balance, stoop, kneel, crouch, or crawl occasionally.
Work Environment Low to moderate noise level; reasonable accommodations available for individuals with disabilities.
Supervisory Requirements Supervises CFY—Speech Clinical Fellows, if applicable, and assists nursing with training of Restorative Aides.
