Pay Range Base pay:
$45.00/hr – $55.00/hr (varies with experience)
Schedule Full Time:
Tuesday – Saturday

Job Overview Grossmont Post Acute Care is seeking a full‑time Occupational Therapist to join an in‑house therapy team. Expect excellent leadership, competitive compensation, a fun work environment, and opportunities for growth and mentorship.
Responsibilities
Screen and evaluate patients with cognitive and/or ADL physical functioning disorders and develop appropriate plans of care per regulatory and clinical practice standards.
Utilize standardized assessments and evidence‑based practice to support clinical interventions.
Deliver rehabilitative, skilled, and medically necessary treatment interventions to patients with cognitive and/or ADL disorders.
Administer various procedures as part of rehabilitation plans, including ADL retraining, therapeutic activities, exercises, modalities, and assistive devices.
Assess patient progress and adjust treatments in the clinical record in accordance with regulatory and clinical requirements.
Evaluate treatment outcomes for effectiveness and modify services based on patient assessment; refer as appropriate.
Provide consultation and counseling to patients, families, caregivers, and other service providers related to physical disorders.
Communicate effectively with supervisors and health team members regarding patient progress, barriers, and treatment plans.
Provide comprehensive discharge summaries of services per regulatory and clinical requirements.
Qualifications
Licensed Occupational Therapist.
Open to OTs at all experience levels.
Strong knowledge of ADL, cognitive, and physical functioning disorders.
Ability to utilize evidence‑based assessments and interventions.
Excellent communication and teamwork skills.
