Physical Therapist (PT) - Mobile Outpatient, Full-time opportunity in Rochester, NH. Responsibilities
The Physical Therapist will primarily provide outpatient rehabilitation services in an in-home setting for the older adult population. The in-home setting may be a patient’s private residence or a room in an independent living facility. Evaluate and treat in the most functional environment (Powerback Rehabilitation To You). Assess needs, develop, and deliver individualized physical therapy programs to facilitate rehabilitation. Identify the need for and make referrals to other disciplines to address comprehensive patient needs. Implement individualized therapy plans designed to restore, reinforce, and enhance performance. Instruct, educate, and train patients and caregivers to promote independence and reduce or correct pathology. Direct activities to promote and maintain health; promote a safe environment and enforce safety standards. Maintain a flexible schedule and work environment across various settings. Supervise Physical Therapy Assistants, temporary licensees, students, and support personnel per state licensure and professional standards (as applicable). Adhere to department and patient treatment schedules, and complete timely clinical and billing documentation. Attend and contribute to patient care and staff conferences; make referrals to other disciplines as needed. Collaborate with rehabilitation team to select appropriate therapeutic regimens and adjust plans of care to individual needs. Conduct screening, examination, and evaluation of patients to establish diagnoses, impairments, and expected outcomes; develop goals and plan of care; document all services. Deliver therapeutic interventions including: therapeutic exercises (aerobic conditioning, balance, gait training, neuromotor development, flexibility, strength, endurance), functional training, manual therapy, airway clearance, integumentary care, electrotherapeutic and mechanical modalities, health and wellness education, chronic disease management, falls risk reduction, case management in home/community, and adaptive devices. Design, fabricate, fit, and train in adaptive devices (including orthotics and prosthetics) as appropriate. Educate patients on mobility activities, evaluate progress, and modify plans of care as needed; perform routine reexaminations to progress treatment. Incorporate health and wellness programming into treatment plans and perform home/community mobility assessments with safety recommendations. Perform other related duties as required.
Qualifications
Must have a valid Physical Therapy license in the state(s) of practice, or proof of license eligibility. Master’s degree in Physical Therapy; or Master’s degree in a Physical Therapy-related field plus a Bachelor’s degree in Physical Therapy; or Bachelor’s degree in Physical Therapy with 5 years of progressive PT experience. Country of origin degree evaluation may be required to confirm U.S. equivalency for degrees earned outside the U.S. Must have a valid driver’s license or reliable, independent transportation to and from patients’ residences.
