Base pay range $56,000.00/yr - $89,000.00/yr Posting Date 11/04/2025 Locations: 4805 Pearl Road, Cleveland, Ohio, 44109, United States; serves patients at Ridge Park Dialysis, Middleburg Heights Dialysis, and Villa of Great Northern Dialysis (T/TH/S). DaVita is looking for a Licensed Social Worker who will be a vital member of each patient’s core care team. This role emphasizes empathy, independence with strong support from the social work team, and advocacy for patients adjusting to dialysis. You will help patients understand their rights and responsibilities, and assist with managing the physical, mental, emotional, and financial demands of End Stage Renal Disease. What You Can Expect Schedule: Monday - Friday 8:00am - 4:30pm (some flexibility/variances based on patient needs) Provide psychosocial health support, interventions as needed, and complete mandatory assessments, screenings, and education Typical caseload around 125 patients spread across three clinics Travel between assigned clinics with a social worker meeting at most once per quarter Build meaningful, long-term relationships with patients and their families in an outpatient setting Collaborate with a team that values work-life balance and positive environment Performance-based rewards based on individual and team contributions Responsibilities As a Licensed Social Worker at DaVita, you’ll be part of the patient’s core care team with emphasis on empathy, independence, and advocacy for patients in dialysis care.
