Overview
The Per Diem Licensed Practical Nurse (LPN) provides direct service and daily nursing interventions in a Youth Community Crisis Stabilization (YCCS) Program for individuals ages 13-18 with behavioral health needs requiring 24-hour care in an unlocked setting. The LPN administers nursing and medications, offers health promotion and access to medical care, and works as part of an interdisciplinary team to support treatment goals through psychoeducation, crisis prevention planning, and skills training. The role helps ensure high-quality standards, a safe and clean environment, and coordinates with multidisciplinary and community providers as needed. The Per Diem LPN Also Assists the YCCS RN with filling prescriber orders, administering medications, and monitoring vital signs. Collaborates with YCCS Direct Service Workers to maintain a safe, recovery-oriented environment. Contributes to assessment, treatment planning, individualized crisis planning, discharge planning, and care coordination for each individual. Implements treatment plans using recovery-oriented, motivational interviewing, harm reduction, trauma-informed, culturally competent, and strengths-based approaches to support safe transitions to home or alternate settings. Provides nursing care, instruction, and assistance with activities of daily living according to each client’s plan. Performs vital signs, including temperature, pulse, respirations, and blood pressure per prescriber orders (pulse oximetry as applicable). Uses crisis intervention techniques when necessary and accesses on-call backup as needed. Completes incident documentation as required. Performs other duties as required. Qualifications
A high school diploma or GED required; completion of an accredited LPN program. Massachusetts LPN license required. Three to five years of supervised practical nursing experience preferred, in clinical or rehabilitation settings with psychiatric conditions. Experience with individuals with psychiatric disabilities, medical illnesses, and co-occurring disorders strongly preferred. Experience with youth and families in community mental health and/or substance use settings is preferred. Ability to work independently and as part of a multi-disciplinary team. Proficiency with computers and electronic health records; strong written and verbal communication skills. Ability to mentor and coach staff at all levels. Must pass multistate CORI, SORI, and background checks, including employment verification and credentialing. Pay and Employment Details
