Nurse Practitioner (NP)
to join our interdisciplinary hospice and palliative care team. The ideal candidate will provide home-based and/or facility-based care to patients with serious illness, focusing on symptom management, quality of life, and supporting patients and families through complex medical decisions.
Position Summary:
The
Hospice and Palliative Care Nurse Practitioner (NP)
play a key role in delivering high-quality, patient-centered care in the home, assisted living, or skilled nursing facility settings. The NP provides direct medical management, symptom control, and emotional support to patients facing serious or life-limiting illnesses. This role requires clinical expertise, compassionate communication, and the ability to collaborate with an interdisciplinary team to promote comfort, dignity, and quality of life.
Key Responsibilities
Responsibilities
Conduct comprehensive assessments of hospice and palliative care patients, including physical, psychosocial, and spiritual evaluations.
Develop, implement, and revise individualized plans of care in collaboration with the interdisciplinary team.
Provide symptom management and prescribe medications as appropriate under the collaborative agreement.
Coordinate with physicians, nurses, social workers, chaplains, and other team members to ensure seamless patient care.
Educate patients and families on disease progression, symptom control, and end-of-life care options.
Document visits, care plans, and interventions in compliance with regulatory standards and agency policies.
Participate in case conferences, quality improvement initiatives, and staff education.
Serve as a clinical resource and mentor for staff.
Provide in-person visit for the medical director or associate for face to face hospice requirements.
Qualifications
Current Nurse Practitioner license in the state of [Insert State]
National certification as an NP (AGNP, FNP, or ACNP preferred)
Minimum 1–2 years of experience in hospice, palliative care, geriatrics, or home health preferred
DEA license (or eligibility to obtain)Excellent clinical judgment, communication, and organizational skills
Comfort with electronic medical records and remote documentation
Reliable transportation and ability to travel to patient homes or facilities (if applicable)
Certification in Hospice and Palliative Care (CHPN, ACHPN)
