Responsibilities

Under general technical and administrative direction of the senior Nutritionist, works independently in a variety of community locations and health centers with professional judgment in nutritional assessment, counseling, education and management of patients receiving health services. Assess individual nutritional status and intervention needs by evaluating laboratory values, growth patterns, dietary histories, medical histories and socioeconomic status of patients. Study and apply federal and state regulations to determine eligibility and provide nutrition services for participants in the WIC Supplemental Food and Nutrition Program. Counsel patients in the WIC program and other health programs on nutritional requirements appropriate to physiological needs; provide referrals to other health department services and community agencies as indicated. Monitor WIC clinics, complete monthly reviews, and document results for the WIC Coordinator. Identify and manage patients requiring intensive or ongoing nutrition treatment for complex problems (e.g., obesity, diabetes, hypertension, anemia) and provide counseling and management; offer nutrition consultation to other health team members. Plan and implement group nutrition education activities in Board of Health clinics, programs and community agencies on topics such as nutrition, economical food purchasing, food preparation and menu planning. Plan and conduct in-service education programs for Board of Health staff and other community agency staff. Develop and prepare nutrition education materials for patients and staff; collaborate with mass media to disseminate accurate nutrition information. Document activities and counseling in medical records using SOAP or equivalent formats; maintain group education records and monthly activity reports. Monitor program effectiveness via internal audits, patient questionnaires, and site visits; manage referrals to other services as needed; collect data to evaluate nutrition program needs in DeKalb County. Perform other related duties as assigned by supervisor. Minimum Qualifications

Education : Bachelor’s degree in dietetics, human nutrition, foods and nutrition, nutrition education, food systems management, or a closely related field from an accredited college or university; verification statement of completion of a Didactic Program accredited by the Academy of Nutrition & Dietetics. Experience : Two (2) years of job-related experience providing nutrition care to groups and individuals; or two (2) years of experience at the lower level Dietitian/Nutritionist 1 (HCP020) or equivalent.
