Journey Hospice RN Case Manager (Per Diem) Position Overview: Journey Hospice has a rewarding and fulfilling position for a per diem RN Case Manager. This position will primarily cover cases in Burlington and Gloucester Counties.
The Hospice RN will oversee a primary caseload of hospice patients and provide skilled, professional nursing care to them, their families, and caregivers. Responsibilities include:
Provide the initial assessment and comprehensive nursing assessment and identification of problems of each patient; reassess nursing needs and collaborate to develop the Hospice Interdisciplinary Team (IDT) plan of care.
Implement the plan of care and reevaluate and revise the plan to meet the patient’s changing nursing needs.
Teach, supervise, and counsel the patient, family members, and staff regarding nursing care and the patient’s needs.
Work closely with other members of the IDT to ensure all patient and family care needs are met.
Provide supervision to Hospice Aides/Homemakers.
Education & Training
Graduate of an accredited registered nursing program.
Bachelor’s degree in Nursing preferred.
Certified Hospice and Palliative Nurse (CHPN) certification preferred.
Certifications & Licensing
NJ RN Licensure by the NJ State Board of Nursing in good standing.
Valid drivers license and reliable transportation insured in accordance with NJ state requirements.
Skills and Knowledge
Maintain a professional attitude and demeanor; communicate effectively and relate courteously with patients, caregivers, and colleagues.
Demonstrate optimism, enthusiasm, and willingness to work constructively with other health care team members.
Support the mission and vision of LSMNJ.
Experience
Minimum of one (1) year general nursing preferred with current hospice, home health, medical, surgical or critical care experience.
Experience with pain and symptom management interventions preferred.
Ability to work within an interdisciplinary setting.
Understanding of Hospice philosophy and principles of death/dying.
