Responsibilities Qualified candidates are eligible for a $1,000 sign-on bonus (half at six months, half at one year anniversary).

Provides for the safety, stability, and well‑being of children and families during the referral period.

Delivers culturally competent, strengths‑based case management services to children and families consistent with evidence‑based practice models.

Decisions are made on guidelines provided by the supervisor, family preservation standard procedures, evidence‑based practice model, and DCCCA organizational policies.

Meets DCF timelines: connects with the family within 24 hours of referral; facilitates initial family meeting within 48 hours of referral; completes assessment‑based case plan within 20 days of referral; and closes case within 3‑6 months of referral.

Engages and works in partnership with families to complete assessments and develop case plan goals and objectives.

Meets with the family in the home weekly, monitoring progress toward case plan goals; and meets monthly with each child individually to assess child safety.

Assesses each child's safety with every family contact and reports all suspected child abuse/neglect; monitors family progress and service provision using assessment tools, and updates family case plan based on those assessments.

Provides weekly home‑based family skills training including nutrition, hygiene, household and time management, home and child safety, basic parenting, transportation, problem solving, and communication to assist parents in providing a safe, nurturing environment for children in their care.

Works closely with the Family Support Worker to provide services for the family and make decisions about child well‑being.

Prioritizes identified problems that require resolution, establishes goals and objectives, utilizes available resources, and arrives at decisions appropriate to the family.

Completes and analyzes initial and ongoing family assessments related to safety, stability, and well‑being, including mental health, substance abuse, education, emotional, social and physical well‑being, parenting and parent‑child relationships.

Utilizes assessments to develop the case plan in partnership with the Family Support Worker/Family Mentor, child/ren, and family; makes community referrals as necessary.

Complies with court requirements: completes and submits timely court reports and appears/testifies as required.

Collects drug screens as necessary; documents case information, including court reports, emails, and activities on a weekly basis.

Responds promptly to family crisis situations, documents incidents, and is available 24/7 to clients.

Requires regular driving to various DCCCA locations and other business locations throughout the day or night as needed.

Promotes teamwork in all activities; fosters and maintains professional relationships with families, community partners, and peers.

Participates in professional development that is relevant to the position.
