Stonehenge American Fork is a post‑acute care facility located in UT that offers long‑term care, short‑term care, and geriatric services. We have an in‑house therapy team and excellent programs for our geriatric clientele.
Responsibilities
Screen and evaluate patients with communication, cognitive, or swallowing disorders, developing care plans that meet regulatory and clinical standards.
Deliver rehabilitative treatment to patients, ensuring compliance with regulatory and clinical practice requirements.
Evaluate treatment outcomes, modify services based on patient evaluations, and make referrals as needed.
Provide consultation and counseling to patients, families, caregivers, and other service providers about speech disorders.
Generate comprehensive discharge summaries in adherence to regulatory and clinical requirements.
Qualifications
Speech‑Language Pathologist license is required.
Open to Speech‑Language Pathologists at all experience levels; new graduates are encouraged to apply.
