Base pay range $75,000.00/yr - $85,000.00/yr

GLIN is seeking a

Nurse (RN) Care Manager

to coordinate the care and provide needed assistance during the immediate post-inpatient/ rehab period of time for patients, their family and/or caregiver across the continuum. Overcoming health care barriers and facilitating timely referrals for appointments to appropriate specialists. Serves as a resource to patients, physicians and others across the continuum of services. Outreach patients telephonically.

Duties And Responsibilities (including But Not Limited To)

To provide education related to diagnosis, treatment, support services and community resources based on disease

To facilitate timely coordination of appointments, tests and procedures communicating any concerns to the appropriate physician/s.

To provide a comprehensive assessment of the patient, family and / or caregivers needs at a medical visit across the continuum, including assessing barriers to care and referring to appropriate disciplines.

To educate the patient, family and/or caregiver on signs and symptoms of complications regarding their specific diagnosis and when to call their PMD.

Other duties as required.

Qualifications or Education, Training, and Experience

Education

Associate of Applied Science in Nursing required

Bachelor of Science in Nursing preferred

Work Experience

Two years' experience in Community based/Primary Care Nursing required

Licenses and Certifications

Registered Nurse License required upon hire

NYS Drivers License required upon hire

Case Management Certification preferred

This position will be hybrid. We offer an outstanding benefits package including health, dental, 401K, vacation, and PTO, as well as a great working environment.

Pay range: $75,000 to $85,000. The referenced pay range represents the minimum and maximum compensation for this job. Individual annual salaries/hourly rates will be set within job’s compensation range, and will be determined by considering factors including, but not limited to market data, education, experience, qualifications, and expertise of the individual and internal equity considerations.
