Psychotherapist – Telos Health Systems
We are looking for experienced Psychotherapists, Licensed Clinical Mental Health Counselors (LCMHC), Licensed Marriage and Family Therapists (LMFT), and Psychologists (PsyD or PhD) to work with the geriatric population in Skilled Nursing and/or Assisted Living facilities.

Schedule Minimum 2 days a week. Flexible fee‑for‑service role, pay per encounter. Prefer non‑consecutive days—choose your own days of the week.
Responsibilities
Perform comprehensive diagnostic assessments of patients’ needs and psychosocial status.
Develop a comprehensive, individualized treatment plan and goals for each patient.
Provide psychotherapy, group, and family therapy when applicable.
Collaborate with members of the facility’s interdisciplinary team to develop treatment goals.
Provide appropriate documentation of all pertinent information obtained in interviews, consultations with other healthcare professionals, referrals, contacts, and supportive services provided to each patient.
Complete necessary documentation and reporting in accordance with departmental standards and Federal and State regulations.
Focus exclusively on therapy and documentation; case management and discharge planning are handled by the billing team and EMR system.
Qualifications
Full licensure required for Psychotherapist roles (LCMHC, LMFT, Psychologist).
Master’s degree required.
