Position is open until filled. Position Summary
The Community Health Representative is responsible for providing assistance and the coordination and development of activities for elderly and physically or mentally disabled Tribal members. Performs community health duties to include patient transport and monitoring, home visits, conducting classes, and providing community education on Tribal health issues. Home Care may include house cleaning, personal hygiene, preparing meals, and other health and wellness related activities. This position is a grade N9; pay rate $20.26/hour. Education And Experience
Must have a High School Diploma or Equivalent. Must have 6 months experience or training in a medical field or with the elderly and physically or mentally disabled. Must have basic computer skills in Microsoft Word, Excel, and Outlook. Must have experience providing personal care, i.e., toileting, bathing, grooming. Must have or be able to obtain experience with medical equipment such as blood glucose monitor, blood pressure cuff/stethoscope, and pulse oximeter. Must have or be able to obtain Certification in First Aid and CPR within 3 months of employment. Must be able to work flex hours and maintain an on-call schedule. Must have a valid Driver’s License for state of residency and be insurable under the Tribal vehicle insurance policy. Must pass a thorough background check including Criminal History, fingerprint check, and pre-employment drug test. Preferred Qualifications
One year’s experience in personal care, patient mobility and transfer, or medical care. Experience giving public presentations and instructing individuals in a class setting. Ability to speak and write the Ute language. C.N.A. (Certified Nurse’s Assistant). CHR Basic 100 Certification. Company
