Nurse Practitioner - PRN

role at

HealthFair Matrix Medical Network is seeking Nurse Practitioners to perform deep health assessments in member homes or facilities, with a focus on evaluating health history, medications, social environment, and other risks to close care gaps through the Matrix Comprehensive Health Assessment (CHA).

Responsibilities Conduct adult/geriatric assessments including medical history, diagnosis and treatment, health education, referrals, and documentation according to Matrix guidelines.

Administer and order point-of-care and lab-based testing as needed.

Collaborate with physicians, case managers, social workers, family members, caregivers, and ancillary staff as appropriate.

Coordinate with the Primary Care Physician on patient education and follow up as necessary.

Provide services at the member’s residence (homes or Skilled Nursing Facilities).

Qualifications Master’s Degree or equivalent experience and NP licensure.

Current NP licensure in the state of practice with prescription authority or ability to obtain it as needed.

Board certification in Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency, or Acute Care.

Current BLS, ACLS or CPR certification.

1 year NP experience preferred; new graduates encouraged to apply.

Valid driver’s license and proof of automobile insurance for residence state.

Strong computer skills and familiarity with medical record software.

Excellent verbal and written communication skills.
