Job Summary

The MoHOP Project Nurse Practitioner will support our DFM mission to ensure access to high-quality, patient-centered, equitable health care for Los Angeles County residents, with a special focus on the medically under-resourced and uninsured racial/ethnic minority populations residing in South Los Angeles’ SPA 6, Skid Row and other regions of Los Angeles County. This position would support the CDU Street Medicine Projects ongoing community outreach and provision of health screening services as well as POC services targeting vulnerable medically under-resourced populations of African Americans and Latinos residing in SPA 6 , Skid Row other regions of LA County in collaboration with community and university partners.

Essential Duties And Responsibilities

Assumes ongoing responsibility for each patient from discovery of the disorder to recovery and referrals to local primary health care facilitates, including MLK Hospital’s Outpatient Clinic and Kedren’s Primary Healthcare Clinic. Performs complete physical examinations, including interviewing patients to obtain complete medical histories as well as ordering, interpreting and evaluating diagnostic tests and examinations. Diagnoses and treats both chronic and episodic disorders including complications of otherwise routine processes such as pregnancy or childhood development. Establishes and documents the health care plan and prognosis, maintaining records of each case that are detailed and complete for any physician reviewing them to evaluate the effectiveness of the plan. Utilizes an Electronic Medical Records system to document all client service interventions/actions. Initiates drug therapy, when a physician is on site and takes responsibility for other reactions as well as therapy when performed prior to consultation with a physician. Provides wound care to the unhoused and other medically under-resourced populations as well as POC screenings including diabetes, HgA1 C, cholesterol, , urinalysis, pregnancy, STI screenings, as appropriate, etc. Determines when the patient has needs immediate referral to a primary healthcare provider. Participates in the Quality Improvement and Assurance activities of Family Medicine Patient Center Medical Home Works in an independent and interdependent relationship with members of the medical staff, which allows for consultation, collaboration, and referral. Works with the CDU Street Medicine Projects clinical team, including physicians and an EMT/RN to coordinate, implement and evaluate service provision Drafts monthly reports

Qualifications/Requirements

LICENSES:

A license to practice as a Registered Nurse and a certificate to practice as a Nurse Practitioner issued by the California Board of Registered Nursing. Applicant's must ensure the License and Certification Section of the application is completed. Provide the title(s) of your required license(s), the number(s), date(s) of issue, date(s) of expiration and the name(s) of the issuing agency for the required license as specified in the Selection Requirements. A current, active certification in accordance with the American Heart Association's Basic Life Support (BLS) for Healthcare Providers (CPR & AED) Program. A valid California Class C Driver License or the ability to utilize an alternative method of transportation when needed to carry out job-related essential functions.

Required license(s) and/or certification(s) must be active and unrestricted, or your application will not be accepted. Additionally, in order to receive credit for license(s) and/or certification(s) in relation to any desirable qualifications, the license(s) and/ or certification(s) must be active and unrestricted.

Knowledge/Ability/Skills

A minimum of two years experience in working with a mobile street medicine project serving under-served and medically under-resourced and uninsured populations of racial/ethnic minorities.

Must relate well to all cultural and ethnic groups in the community Ability to work and communicate well with others, particularly in a clinical Demonstrate sensitivity towards marginalized Skills and ability developing individual service plans, prioritizing work flow and resolve Bi-lingual in Spanish

The ability to perform highly detailed work with sustained attention and care while providing/obtaining information on numerous inquiries is paramount to success in this position.
