Fountains at Mill Cove is seeking a compassionate and skilled
Per Diem Speech-Language Pathologist
to join our interdisciplinary team at our in-house subacute rehabilitation facility in Jacksonville, FL. The ideal candidate will provide high-quality, patient-centered care to individuals recovering from illness, injury, or surgery, helping them regain their communication, cognitive-linguistic, and swallowing abilities. Key Responsibilities
Conduct comprehensive evaluations of patients with speech, language, cognitive-communication, and swallowing disorders. Develop and implement individualized treatment plans based on evaluation findings and patient goals. Provide therapy services in accordance with best practices and facility policies. Document patient progress and outcomes accurately and in a timely manner using EMR systems. Collaborate with a multidisciplinary team, including physicians, nurses, physical and occupational therapists, and case managers, to support optimal patient recovery. Educate patients, families, and caregivers on treatment strategies and carryover techniques. Participate in discharge planning and recommendations for continued care as appropriate. Qualifications
Current Florida SLP license (or eligibility) Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP) preferred; CFY candidates may be considered Prior experience in subacute rehab, skilled nursing, or post-acute care preferred CPR certification (or ability to obtain) Strong clinical skills and ability to work independently Excellent communication, organizational, and interpersonal skills Position Details
Type: Per Diem (as needed; flexible scheduling) Setting: On-site, subacute rehab unit Shifts: Weekdays and/or weekends available based on facility needs Why Join Us
