Job Summary Recommend and assist with access to medical, educational, social, vocational, housing, and more to support program goals. Initiate referrals to the next level of care, and coordinate with physical health providers to ensure whole-person care, and advocate in the best interest of clients.
Responsibilities Reasonable accommodation may be made for individuals with disabilities to perform essential job functions.
Provide direct services including behavioral health assessments, therapy (couples, individual, and group), case management, care coordination, care plan development, and crisis intervention.
Work with an interdisciplinary team to provide wrap around services.
Maintain a caseload of clients as assigned by the Lead and Program Manager.
Ensure accurate and timely documentation and meet the minimum requirements for billable services.
Engage in office, telehealth and field-based services, as needed.
Maintain confidentiality of clients care.
Participate in scheduled program meetings.
Maintain board registration that is required to bill for services provided to clients.
Participate in weekly supervision meetings as assigned by the Clinical Supervisor, if applicable.
Complete regular training, as assigned by the agency, county, or Program Manager.
Other relevant duties as assigned.
Qualifications
Masters degree and CA BBS registration (AMFT, ASW, APCC) or license (MFT, LCSW, LPCC) required.
Valid CA drivers license and current auto insurance per contract requirements required.
Experience providing mental health and/or substance use disorder services preferred.
Cultural competency understanding required.
Bilingual preferred.
Knowledge of Microsoft Office and EHR software preferred.
