Your Contribution to Our Success A Medical Care Manager contributes to AHF by utilizing their experience, skills, and education together with a thorough understanding of the Ryan White Care Act Program within their specific area. Working with our providers and healthcare center team, you will ensure that every Ryan White client receives the support they require and access to available resources.

You will meet clients, complete Needs Assessments, develop Action/Care Plans, and provide ongoing education to patients so that they understand their diagnoses and medications. For Ryan White contracts that combine psychosocial needs within Medical Care Management, you will address basic needs such as housing, food, and clothing and facilitate referrals to programs and agencies serving those needs.

As a passionate advocate for our top initiatives, you will help bring the essence of the Ryan White Care Act—the safety net for uninsured HIV/AIDS patients—to our most vulnerable population. Your work will directly contribute to AHF’s success in meeting the deliverables of our Ryan White contract and to broader measures established by the HIV/AIDS Bureau and the Department of Health and Human Services.

Licenses & Certifications

BS/BA in Social Work, or other social service field with two (2) years of direct service delivery.

Familiarity with housing case management.

Familiarity with housing entitlement systems and procedures.

HOPWA Training Modules.
