Case Manager with a housing focus through the 1115 Waiver Program , you’ll take on the meaningful role of Housing Advocate for Catholic Charities of Delaware, Otsego, & Schoharie Counties. You’ll be the go-to support for individuals facing housing challenges—helping them find stability, connect with vital resources, and take steps toward brighter, more independent futures. Responsibilities

Use community resources to assist residents in meeting their financial, medical, employment, or educational needs. Coordinate and act as a liaison between community agencies and the residents. Ensure all resident benefits are activated prior to admission and remain active for the duration of their affiliation with Catholic Charities. Obtain resident food stamp benefits monthly for the agency. Assist residents with all paperwork relating to benefits and attend any re-certifications that are scheduled.

Qualifications

Associates Degree in Human Services or related field 1-3 years of experience in mental health, substance use, or human services

Compensation and Schedule

Case Manager Minimum Salary: $19.00 / Hour Case Manager Maximum Salary: $20.00 / Hour Case Manager Schedule: Monday – Friday 9am - 5:30pm (40 hours a week)
