Job Description
Nursing Program Coordinator who coordinates the development and efficient operation of specialized clinical innovation programs in the Division of General Internal Medicine within the Department of Internal Medicine; coordinates activities and services provided by the programs; supports development and assist with implementation of program goals and objectives; participates in evaluating program progress and outcomes and identifies quality improvement areas; responds to and ensures adherence to a diverse set of program funder requirements to include preparation of comprehensive quarterly and year-end progress reports that impact future funding; provides ongoing assessment of patients and their families regarding treatment, complications, and general health maintenance; develops and provides educational programs, materials and information; monitors and reports safety issues; serves as a resource for care management for patients, referring physicians and specialty services and staff; prepares presentations and provides education to professional health care providers; serves as an advocate for appropriate patient care with insurance carriers, professional providers of care, legislators, and other entities; Responsibilities and Requirements
