Hospice Licensed Social Worker , you will conduct psychosocial assessments, offer counseling and emotional support, and coordinate care planning with our interdisciplinary team (IDT). Your guidance will help patients and families navigate medical, emotional, financial, and spiritual challenges during the end-of-life journey. Key Responsibilities: Provide psychosocial assessments, direct casework, and counseling services to patients and their families. Participate in the preparation, evaluation, and execution of the interdisciplinary care plan to assure that the aims of continuity and comprehensiveness of care are accomplished. Work collaboratively as a member of the Interdisciplinary Team(s). Prepare clinical and progress notes for the clinical record per Hospice, state, and federal guidelines/standards. Prepare documentation for the clinical record per Hospice, state, and federal guidelines and standards. Maintain and update knowledge of community resources. Facilitate referral of patients/families to those resources as needed and desired. Participate in taking on-call responsibilities as necessary, including weekends and evenings. Participate in the Interdisciplinary Team meetings, staff meetings, and other meetings as requested. Qualifications: A Master's degree from an accredited educational institution of social work is preferred, or another related clinical degree. Licensed Social Worker (LSW)
in the State of New Jersey (required) Healthcare experience, preferably in hospice, palliative care, gerontology, or behavioral health In-depth understanding of
