Transplant Coordinator II, RN - Transplant Call Team - Full Time 12 Hour Rotating Shift (Exempt) (Union) | Keck Medicine of USC | Hospital | Los Angeles, California
Responsibilities
Develops and maintains nursing practice standards of care for a specific transplant population. Serves as a liaison and provides consultation and education services to support standards of care. Identifies and resolves clinical issues relating to the transplant patient. Collaborates with staff, the patient, and family concerning patient evaluation and discharge planning. Maintains appropriate documentation for transplant candidacy and post-transplant follow-up care.
Coordinates communications with the outpatient transplant population (such as transplant clinic visits, community referrals, telephone consultations, outpatient treatments, and subsequent hospital admissions).
Develops and coordinates a system to organize the flow of patients from initial referral, through evaluation and education, to post-transplant follow-up and referral back to the community.
Participates in collaborative, interdisciplinary research activities related to transplant patients or process improvement activities. Rotating on-call responsibilities as needed, including nights, weekends and holidays.
Assists with on-call duties, including delegating tasks to the Donor Allocation Specialist, reviewing deceased donor organ offers, triaging patient calls, and coordinating donor organ acceptance/refusal and organ recovery in coordination with transplant surgeons and related teams.
Coordinates urgent transplant listings after hours and on weekends, including evaluation of consults, labs, and procedures. Provides complex logistical support for organ recovery and placement and ensures timely UNOS-related responsibilities.
Maintains professional communication with surgeons, potential recipients, donation staff, and donor hospital staff. Triages calls after hours and escalates to the on-call physician when appropriate.
Demonstrates knowledge in the care of patients with renal, hepatic, cardiac, and respiratory failure, transplantation-related disorders, and immunosuppression management, including adverse reactions and drug interactions. Understands UNOS policy and CMS regulatory requirements related to organ allocation and waitlist management.
Ensures completion of UNOS responsibilities (listing, delisting, disease transmission reporting, donor culture reports, status updates for solid organs) and collaborates with wait-list management teams. Operates within the program using Cerner and UNOS platforms and maintains recipient charts and UNOS donor data.
Provides 24/7 coverage on a rotating schedule, including nights, weekends and holidays; may be a home-based position with on-campus meetings/trainings as determined by Donor Allocation Managers.
Required Qualifications
Bachelor's degree in Nursing or Associate's degree in Nursing with BSN to be conferred within two years of hire if applicable
5 years of clinical experience in organ donation or transplantation
Excellent communication skills in English (speaking, reading, writing)
Knowledge of medical procedures and conditions; ability to perform duties including blood collection and apheresis procedures in accordance with policies and procedures
Required Licenses/Certifications
Registered Nurse – RN (California)
Basic Life Support (BLS) Healthcare Provider – American Heart Association
Fire Life Safety Training (Los Angeles City) – if no card, must obtain within 30 days of hire and maintain renewal
Certified Clinical Transplant Coordinator (CCTC) or CPTC; if CPTC is held, must obtain CCTC within three years of hire
Salary: The annual base salary range for this position is $145,600.00 - $237,120.00. USC considers factors such as scope, responsibilities, experience, education/training, skills, equity, and market conditions when extending offers.
