Housing Works was founded in 1990 with a long‑term commitment to AIDS advocacy. The organization operates New York State’s first harm‑reduction‑based, OASAS‑licensed outpatient drug treatment program, in addition to health‑home care management, behavioral health, syringe exchange programs, 10 high‑end thrift shops in Manhattan and Brooklyn, and a beloved Bookstore Café in Soho.
Compensation:
$19/hr–$20/hr, commensurate with experience Overview
The Intake Specialist will screen patients for insurance eligibility, register new patients, complete daily sliding‑fee‑scale applications for uninsured patients, issue Good Faith Estimates for clients who opt to self‑pay, and assist the Patient Care Coordinator with medical case‑management services for all Primary Care patients. Responsibilities
Complete New York State (NYS) Assistor Training for Insurance Enrollment within the first 30 days. Enroll uninsured patients with insurance through the NYS Market Place, including ADAP applications for Health Services Clients. Issue Good Faith Estimates at least 3 days in advance of the visit. Verify insurance for daily and walk‑in visits. Complete intake screening of new patients and determine program and insurance eligibility for ADHC, OASAS, Health Home, OMH, PrEP, RAD, etc. Complete PRAPARE screening for new and annual patient visits. Generate lead in eICare for new patients, including MRN. Submit sliding‑scale application for uninsured/underinsured patients and upload to the EMR. Utilize the PRAPARE tool and referral services for all Primary Care clients, coordinating care and case‑management referrals. Assess primary care patients for entitlements and assist those who need to enroll in Medicaid HMO; help clients change their insurance or PCP. Participate in interdisciplinary meetings and health fairs for insurance enrollment. Attend the Morning Huddle and regular medical staff meetings, including Quality Improvement activities. Document clinical interventions such as PHQ‑2/9 in the EHR. Perform outreach by telephone and written letters to patients who have not accessed services or missed appointments for over three months; document outreach efforts in the medical record. Support and participate in advocacy efforts to end homelessness and AIDS crises. Assist with patient registration, ensuring professionalism and accuracy. Schedule patient appointments efficiently. Provide exceptional customer service, resolving problems and escalating to Clinic Manager or Health Center Director as needed.
