About The Program
Transitional Housing Program: Provides housing and supportive services to non‑minor dependent pregnant and/or parenting young women ages 18‑21 in the foster care system, and to those 18‑24 who have transitioned out of foster care or the juvenile justice system.
Role Summary Under the supervision of the Lead Family Advocate, the Family Advocate conducts basic social casework and coordinates related parent‑involvement services, assisting youth in setting personal goals around housing, employment, education, and mental health. Advocates guide families on education, development, health, nutrition, disability, and other services, serving as the primary point person for family case management.
Schedule: Tuesday‑Friday, 12:30pm‑10:00pm; every other Saturday off, 10:30am‑7:00pm
Location: 155 N. Occidental Blvd, Los Angeles, CA 90026
Duties And Responsibilities
Conduct family assessments, identifying strengths, interests, and needs.
Support clients through the housing stability process, including document preparation, housing search, landlord engagement, and referrals to community resources that align with client goals.
Complete thorough case notes and service entry data for CLARITY in a timely manner per SHP policies.
Participate in case conferences, meetings with collaborative agencies, and community meetings; available for impromptu conference calls as needed.
Develop a self‑sufficiency plan with each resident outlining objectives, action steps, timeframes, and supportive services.
Qualifications
Bachelor’s degree in Behavioral Science, or Master’s degree in Behavioral Science or Human Services (per current contract guidelines).
Minimum of two years of experience with transition‑aged youth who have transitioned out of foster care or the juvenile justice system.
Valid California driver’s license with an insurance‑acceptable driving record.
