Position Summary: As an independent lease holder in Walmart stores, you'll launch your services and take a leading role in patient care for eye, vision, and general health. You will establish your schedule, hours, and fee structure, and keep 100% of exam and professional fees.

Control your patient schedule
Set your fee schedule
Keep 100% of exam/professional fees
Optional Options for a fair market value
Equipment/Instrumentation
Administrative and/or Opt-Tech support services (if state allows)
Minimum Qualifications
Doctor of Optometry (OD) from an accredited optometry school and valid state optometry license
Ophthalmologist (MD or DO) from an accredited school, valid state ophthalmology license, General Liability Certificate, Professional Liability Insurance Certificate, and W-9
ME & VA license agreements are with Gumberg Asset Management Company.
