Overview The primary purpose of this position will be to provide Adult Services programs including but not limited to: Representative Payee, Special Assistance In-Home, In-Home Aide, Placement Services, and General Assistance to clients that are eligible for these services. This position provides assessments of an individual’s needs to develop service plans, which include supportive services to enable the adults to remain safely in their homes.

Responsibilities

Provides case management services on an individual basis to assess the household needs and determines the goals needed to be eligible to receive the Work First cash assistance and supportive services for the family.

Provides coverage for Adult Services Intake, assisting customers both in person and via telephone with making connections in the community related to housing, education and other pertinent resources to assist with individual and family needs.

Responsible for making referrals to other units in the agency or various community resources as necessary.

Education & Experience Master’s degree from an accredited school of social work; or a bachelor’s degree from an accredited school of social work and one year of directly related experience in case management, assessment & referral, supportive counseling, intervention, psycho‑social therapy and treatment planning; or a four year‑degree in a human service field and two years of directly related experience in case management, assessment & referral, supportive counseling, intervention, psychosocial therapy and treatment planning; or graduation from a four‑year college or university and three years of directly related experience in case management, assessment & referral, supportive counseling, intervention, psycho‑social therapy and treatment planning; or an equivalent combination of training and experience.

A copy of a certified college transcript and a letter of reference must be provided upon request.

Computer Skills Proficient in MS Office (Excel, Word and PowerPoint Experience).

License & Credentials Possession of a valid N.C. driver’s license is required for this position and maintaining a driving record in accordance with Alamance County policy.

Other Position Information This position is subject to reallocation contingent upon funding, agency needs and client population and may be required to staff shelter overnight.
