Pay Range Base pay range: $85,000.00/yr - $160,000.00/yr.
Role Overview Under the supervision of a Physical Therapist, you will assist in providing appropriate aspects of quality patient care and address the needs of individuals in their homes, so they can live safely and productively in their home environment.
Responsibilities
Examine each individual and develop a plan using treatment techniques to promote ability to move, reduce pain, restore function, and prevent disability.
Work with patients to prevent the loss of mobility before it occurs by developing fitness‑ and wellness‑oriented programs for healthier and more active lifestyles.

Qualifications
License PTA in California.
Home Health experience highly preferred.
Current CPR License.
Current CA Driver’s License.
Computer proficiency.
Strong organization and communication skills.
Works well independently and in a team.
Excel at building rapport with patients and providing the highest quality of care.
Experience with high acuity patients is a plus.
Experience working with an interdisciplinary team is highly desired.
Case Management fundamentals highly desired.
Self‑directed.
Empathetic.
Critical thinker.
Outstanding interpersonal skills.
Leadership skills.
Organized.
Must complete a computer literacy and competency assessment prior to interview.
Additional Information * You will be assigned a primary location; however, you may occasionally be asked to provide coverage outside that area.
