We are seeking skilled Physical Therapists to join our actively growing Home Health Team in St. Louis City and County, St. Charles County, Warren, Lincoln, Franklin, and Jefferson Counties. Cooperative Home Care is a locally owned and operated home care company. The role involves covering clients in a set territory with company car and gas card provided.
Pay Pay is calculated per type of visit.
Responsibilities
Review and evaluate physician's or podiatrist's referral and patient's medical record to determine physical therapy required.
Plan and prepare a written treatment program based on the evaluation of available patient data.
Perform patient tests, measurements, and evaluations, such as range-of-motion and manual muscle tests, gait and functional analyses, and body parts measurements, and record and evaluate findings to aid in establishing or revising specifics of treatment programs.
Plan and administer prescribed physical therapy treatment programs for patients to restore function, relieve pain, and prevent disability following disease, injury or loss of body part.
Administer manual therapeutic exercises to improve or maintain muscle function, applying precise amounts of manual force and guiding patient's body parts through selective patterns and degrees of movement.
Administer treatment involving application of physical agents, such as heat, light, cold, water and electricity. Administer traction and massage. Evaluate, fit and adjust prosthetic and orthotic devices and recommend modifications to the orthotist/prosthetist.
Observe, record, and report to the physician or podiatrist the patient's treatment, response and progress.
Instruct other health team personnel, including, when appropriate, home health aides and family members in certain phases of physical therapy with which they may work with the patient.
Instruct patient and family in total physical therapy program.
Prepare clinical and progress notes for the clinical record.
Review physician's referral and patient's medical records to help determine diagnosis and physical therapy treatment required.
Obtain patients' informed consent to proposed interventions.
Discharge patient from physical therapy when goals or projected outcomes have been attained and provide for appropriate follow-up care or referrals.
Provide information to the patient about the proposed intervention, its material risks and expected benefits, and any reasonable alternatives.
Inform patients when diagnosis reveals findings outside physical therapy and refer to appropriate practitioners.
Direct, supervise, assess, and communicate with supportive personnel.
Provide educational information about physical therapy or physical therapists, injury prevention, ergonomics, or ways to promote health.
Requirements
Proven experience as a Physical Therapist
Valid license to practice physical therapy in the state of Missouri
Strong knowledge of anatomy, physiology, and rehabilitation techniques
Excellent communication and interpersonal skills
Ability to work effectively in a team environment
