Speech Language Pathologist (SLP) - Nursing Home Setting

$55-$63 per hour

We are currently seeking a
Speech-Language Pathologist (SLP)
to join our rehabilitation team in a Skilled Nursing Facility setting. The ideal candidate is passionate about improving the quality of life for our residents through patient-centered care and collaboration with a supportive interdisciplinary team.
Responsibilities
Conduct evaluations and re-evaluations for residents with speech, language, cognitive, communication, and swallowing disorders.
Develop and implement individualized treatment plans in accordance with resident goals and clinical findings.
Collaborate with nursing, dietary, and therapy teams to ensure coordinated care delivery.
Document all services provided in accordance with federal, state, and facility guidelines.
Educate residents, families, and staff on communication strategies and dysphagia precautions.
Qualifications
Master’s degree in Speech-Language Pathology from an accredited program.
Current SLP license in the state of New York.
Certificate of Clinical Competence (CCC-SLP) preferred; CFY candidates are also encouraged to apply.
Previous experience in a Skilled Nursing Facility or long-term care setting is preferred.
Strong clinical, organizational, and communication skills.
