Ahava Therapy Services provides Physical, Occupational, and Speech Therapy services and is committed to delivering innovative, evidence‑based care through teamwork and interdisciplinary collaboration. Our goal is to maximize patients' potential while improving their quality of life.
Pay Range Base pay range:
$80,000.00/yr - $100,000.00/yr
(actual pay will vary based on skills and experience).
Responsibilities
Review available resident information and evaluate the impact of this information on the assessment and treatment process.
Conduct and document a thorough functional resident assessment utilizing standardized procedures.
Develop and implement an individualized treatment plan based on resident needs and containing achievable functional goals.
Comply with evaluation, treatment, and documentation guidelines and consistently maintain timely and accurate department records.
Instruct assistants in treatment procedures and provide close supervision while they work with residents.
Enter treatment and progress notes in the resident record following each treatment session.
Revise treatment plans as needed throughout the treatment process.
Document the course of treatment in a clear, concise discharge summary utilizing a prescribed format.
Orient residents to program services and treatment procedures appropriate to their needs.
Work with nursing staff, residents, and families to maximize residents' functional abilities.
Instruct residents and family or other caregivers about exercise programs and therapeutic procedures to continue at home after treatment.
Assist in the referral process when a resident requires additional services after discharge and maintain knowledge of community resources.
Attend and participate in rehabilitation team conferences regarding resident progress, problems, or needs.
Attend and participate in other department/facility meetings as required.
Access continuing education opportunities appropriate to discipline and responsibilities to maintain clinical competency.
Participate in the quality improvement process.
Act in compliance with Ahava Therapy Services' regulatory and professional standards and guidelines.
Adhere to facility policies and procedures and participate in facility quality improvement and safety programs.
Perform other duties as assigned and required to provide quality care to residents.
Understand reimbursement by various payers and allocate therapy resources efficiently by resident goals and outcomes related to discharge destinations.
Know OBRA regulations, HIPAA, Local Coverage Determination, and third‑party reimbursement guidelines.
Support the company's mission, vision, and project a professional image.
Independently perform duties and recognize situations where direct supervision is needed; willingly help others.
Maintain the expected productivity level discussed for your position at your facility.
Qualifications
Bachelor's or Master's of Science degree in Physical Therapy.
Current license in the state of practice.
Must have BLS CPR certification on hiring or obtain it during orientation; certification must remain current.
