Patient Assessment: Evaluate patients’ physical conditions, mobility, and functional abilities to develop comprehensive treatment plans tailored to their needs. Treatment Planning: Create and implement personalized therapy programs that address specific goals, improve mobility, alleviate pain, and enhance overall physical function. Therapeutic Interventions: Administer therapeutic exercises, manual therapy techniques, and modalities to help patients achieve their rehabilitation goals. Progress Monitoring: Regularly assess and document patient progress, adjusting treatment plans as necessary to optimize outcomes. Patient Education: Educate patients and their families on exercise techniques, safety measures, and home care strategies to support ongoing recovery and maintenance. Collaboration: Work closely with physicians, nurses, and other healthcare professionals to ensure coordinated care and address any issues that may impact patient progress. Documentation: Maintain accurate and timely documentation of patient evaluations, treatment plans, progress notes, and discharge summaries in compliance with facility policies and regulatory requirements. Compliance: Adhere to all safety and infection control protocols, as well as facility policies and procedures, to ensure a safe and effective therapeutic environment. Qualifications
Education: Doctoral or Master’s degree in Physical Therapy from an accredited institution. Licensure: Current state licensure as a Physical Therapist. Experience: Previous experience in a skilled nursing or long‑term care setting preferred, but not required. Personal Attributes: Compassionate, patient‑focused, and dedicated to providing high‑quality care. Strong organizational skills and attention to detail are essential. Skills
