to join our therapy team at our newly acquired senior community in Sauk City, WI. This role offers flexibility for full‑time, part‑time, or PRN schedules—an excellent opportunity for a dedicated clinician to make a meaningful difference in the lives of residents while working in a supportive, therapist‑led environment.

Your Responsibilities
Provide compassionate and effective physical therapy services to adult and geriatric residents.
Develop and implement individualized treatment plans to improve mobility, strength, balance, and functional independence.
Support residents in achieving personal goals and rehabilitation milestones.
Accurately document care and maintain compliance standards (experience with Net Health a plus).
Collaborate with therapy and nursing teams to ensure patient‑centered outcomes.
Educate residents and families on safe techniques and strategies to maximize independence.
Participate in team meetings and contribute to care planning.
What You Bring
Current Wisconsin PT license.
Strong clinical skills and a passion for working with older adults.
Excellent communication and teamwork abilities.
Flexibility, professionalism, and a positive attitude.
