What you will do in this role
Conduct one‑on‑one, group and family psychoeducational and process group sessions.
Develop treatment plans and attend treatment planning meetings and reviews on an ongoing basis.
Assess environment and patient’s safety status and report/respond to actual and potential safety issues or changes in the patient’s condition.
Identify risk factors, screening criteria, and interventions for patients with special precautions (e.g., suicide, homicide, falls, elopement).
Complete assessments, group notes, and all other documentation promptly and in compliance with requirements, as appropriate to scope.
Collaborate with the multidisciplinary team to assess and coordinate patients’ discharge planning needs and readiness for discharge.
Conduct post‑discharge follow‑up as indicated.
Qualifications
Basic Cardiac Life Support must be obtained within 30 days of employment start date.
Nonviolent Crisis Intervention must be obtained within 90 days of employment start date.
Fingerprint required.
Masters degree in a related field.
Licenses: LPC/LMHC, CTRS, LMSW/RCSW‑I, LPC‑A/MHCI, LMFT, LCSW (any of the listed credentials).
