The Opportunity: Overview: As a newly hired .6 FTE Nurse Practitioner you will receive a signing bonus of $1,500. This bonus will be paid in one lump sum on the next regularly scheduled pay date after you start employment with Multnomah County. Furthermore, there will be a retention bonus of $1,200 after your 1-year anniversary date. This signing bonus is taxable, and all regular payroll taxes will be withheld. Additional Compensation: Corrections Health Nurses receive an additional 10% hourly premium and hourly differentials for working swing ($2/hr), graveyard ($5/hr), weekends ($3/hr) shifts. Benefits:

Transition Team Nurse Practitioner

is to provide comprehensive nursing care and treatment for justice-involved individuals within Multnomah County. The Transition Team Nurse Practitioner will be under the clinical supervision of the Corrections Health Medical Director and will function as the main medical provider for the Transition team; guiding clinical care with a focus on medication-supported recovery and harm reduction, and collaboration with Health Department and community providers. As a Transition Team Nurse Practitioner in Corrections Health, you will help serve in The Multnomah County Detention Center (Downtown Portland), Inverness Jail (By PDX Airport) and the Mead Building (Downtown Portland). As the

Transition Team Nurse Practitioner , other duties will include: Independently assess clients with review of systems and health history and identify normal/abnormal findings. Developing a problem list and therapeutic teaching plans. Ordering and reviewing appropriate diagnostic tests and medication. Recording all pertinent data in approved clinical SOAP format. Referring clients to the Physician when beyond scope of practice or Nurse Practitioner (NP) laws. Case managing difficult clients as “primary providers” when indicated. Training and precepting staff and students, offering consultation to internal staff and external providers. Providing consultation to nursing staff, Health Department Clinics, other facilities and community providers as indicated for continuity of client care. Assessing and counseling patients regarding preventive care needs; delivers services or makes appropriate referrals. Researching programs and support systems in the community to provide information and resources for those transitioning to independent or community group living situations. Assisting and advising staff with difficult and complex medical, social and behavioral health issues. Consulting with leadership, identifying partners and forging key strategic partnerships with diverse key internal and external stakeholders. Completing patient documentation and managing electronic in-basket according to established policies and procedures. Working in an effective, respectful and culturally competent manner with team members, community partners, and clientele from culturally/ethnically diverse communities. TO QUALIFY:

page. Minimum Qualifications/Transferable Skills*: Education/Training: Master's Degree in Nursing AND one (1) to four (4) years of experience that demonstrates the ability to perform the duties of the position.

Certification Requirements: License required to practice medicine as a Nurse Practitioner as outlined by Oregon Revised Statutes

(ORS) 678.375 CPR certification required

Other: Passing a Multnomah County Sheriff's Office background check is required.
