Overview
Conduct thorough intake assessments and complete documentation within 48 hours Gather and evaluate clinical and psychosocial data from multiple sources Utilize motivational interviewing to assess client readiness and engagement Educate individuals and families on available therapeutic services and supports Initiate person-centered planning in alignment with Medicaid guidelines Develop safety plans as appropriate based on identified risk factors Coordinate referrals for psychological testing or additional evaluations when needed Promote group and support service options when beneficial Participate in clinical supervision, case consults, and team meetings Accurately document all client interactions and maintain compliance standards Complete required training (25 hours annually) and maintain current licensure
Required Qualifications
Master’s degree in Social Work, Psychology, or Counseling – Required Active State of Michigan licensure (LMSW, LLMSW, LPC, etc.) – Required Minimum 1 year of post-master’s experience working with adults with serious mental illness – Required Must obtain QMHP and QIDDP credential within 120 days of hire – Required Must have CAADC or be on an active, approved MCBAP Development Plan within 6 weeks of hire Ability to build strong therapeutic relationships with clients and families Skilled in person-centered treatment planning and transitioning clients to long-term care
