Suffolk County, NY!

Daily Responsibilities for Care Transitions Nurse Manager – RN :

Will make field visits but will not carry their own caseload. Required Qualifications for Care Transitions Nurse Manager – RN :

Active NY Registered Nursing license 3+ years' experience as a Registered Nurse / RN 2+ years of recent experience at a Certified home health agency (CHHA) 1+ year of recent supervisory experience

Pay Details:

$135,000 – $140,000 annually
