This role supports licensed Physical Therapists in delivering high-quality care across the home health setting. This position offers a flexible schedule, making it an excellent opportunity to supplement your current income or increase your overall earning potential. Currently we are covering the areas around and between Great Neck to Floral Park and Bayside to Queens Village. Clinical Care & Patient Interaction Implement individualized treatment plans under the supervision of a licensed PT. Provide therapeutic exercises and functional training as directed. Educate patients and caregivers on home exercise programs and discharge instructions. Ensure a safe, clean, and supportive environment for all patient interactions. Communication & Coordination Communicate effectively with supervising PTs and home health agencies at evaluation, reassessment, discharge, and when concerns arise. Attend care conferences and collaborate with interdisciplinary teams to support optimal outcomes. Documentation & Scheduling Monitor patient progress and document visits accurately within 1–2 days. Manage your own schedule and patient caseload efficiently. Complete visit invoices and other administrative requirements in a timely manner. Pay: $75,000.00 - $125,000.00 per year **Applicants must be within Queens or Nassau County in New York, USA **1-2 year experience in home health preferred** **Training will be provided** Seniority level
