Clinical Care Coordinator (C3)
proactively identifies and removes barriers that may impede department or system wide patient flow by serving as a consultant and educator with medical staff regarding patient status, bed placement, payor requirements and utilization of resources. The position (Unit C3) will act as the primary liaison between emergency and surgical services, nursing, behavioral health, finance, physicians, utilization review to ensure appropriate bed is assigned, patient care needs are met and treatment plans executed at the appropriate levels. Will work collaboratively with all members of the healthcare team to improve patient throughput and length of stay (LOS) performance of the organization.
Responsibilities
Proactively identify and remove barriers to patient flow; serve as consultant and educator to medical staff regarding patient status, bed placement, payor requirements, and utilization of resources.
Act as the primary liaison between emergency and surgical services, nursing, behavioral health, finance, physicians, and utilization review to ensure appropriate bed assignment and that treatment plans are executed at the appropriate levels.
Collaborate with all members of the healthcare team to improve patient throughput and LOS performance.
Qualifications
Bachelor's Degree in a health-related field is required
Three (3) or more years of clinical experience is required
Nurse Case Management experience is preferred
Registered Nurse in the State of Georgia required
Basic Life Support (BLS) Certification is required
For Critical Care and ECC units Advance Cardiac Life Support (ACLS) is required
Certified Case Manager (CCM) or American Case Manager (ACM) Certification is preferred
