Job Summary
The
Clinical Care Coordinator (C3)
proactively identifies and removes barriers that hinder department or system-wide patient flow. Serves as a consultant and educator with medical staff regarding patient status, bed placement, payer requirements, and utilization of resources. Acts as primary liaison between emergency and surgical services, nursing, behavioral health, finance, physicians, and utilization review to ensure appropriate bed assignment and patient care needs are met. Works collaboratively with the healthcare team to improve patient throughput and length of stay (LOS) performance of the organization.
Job Qualifications
Bachelor's Degree in a health-related field is required
Three (3) or more years of clinical experience is required
Nurse Case Management experience is preferred
Registered Nurse in the State of Georgia required
Basic Life Support (BLS) Certification is required
For Critical Care and ECC units Advanced Cardiac Life Support (ACLS) is required
Certified Case Manager (CCM) or American Case Manager (ACM) Certification is preferred
