Job Title: Teacher/Counselor Department: Residential Programs Hours: 40 hours per week; schedule includes 2pm-10pm on Mondays and Wednesdays, 9am-5pm on Tuesdays, Thursdays, and Fridays. Mission
Advocacy Empowerment Opportunity Responsibilities
Promotes optimal health for individuals through routine, preventive and emergency health care. Ensures that residential environments are safe, secure and where individuals are free from harm. Provide education, training, counseling, and physical assistance to achieve ISP goals in areas such as functional academics, domestic skills, health and personal hygiene, social and leisure skills, communication, community mobility, self-esteem, and money management. Develop, write and present annual ISPs and semiannual assessments in consultation with other residential staff and Director. Provide transportation to individuals as necessary. Communicate with family, guardians, funding sources, advocates, volunteers, and employers to facilitate consistency of programming and support. Perform case management duties including maintenance of confidential files, documentation of activities and training; ensure cases are reviewed. Demonstrate computer skills and learn agency programs for documentation of data. Attend regular meetings and perform other duties as needed. Note: This role has no supervisory responsibilities. Qualifications
Education and/or Experience: BA/BS in Human Services or related field, or an associate degree with 3 years related experience with persons with ID/DD, or HS Diploma/GED with 5 years related experience. Language Skills: Ability to read, understand instructions and memos; ability to write simple correspondence; ability to present information to residents, families, DDS and colleagues. Mathematical Skills: Proficiency with basic arithmetic and ability to interpret data and graphs. Reasoning Ability: Capacity to follow written or oral instructions and handle problems with multiple variables. Vehicle and Travel: Requires legally insured and inspected automobile for use during working hours; program vehicles may be used. Certificates, Licenses and Registrations: Valid driver’s license; Medication Administration, First Aid, CPR certifications as required. Physical Demands: Regularly use hands; stand, walk, sit, reach; may lift up to 50 pounds; vision requirements as specified. Work Environment: Exposure to outside weather; moderate noise typical of residential settings; ability to assist residents during meals and breaks as needed. Job Functions
