Location: Riverside, CA | Travel: Up to 80% | Work Type: Regular | Schedule: Full Time | FLSA Status: Exempt | Salary Range: $68,000 - $75,000
Summary: The Enhanced Care Management (ECM) Program Manager provides leadership and direction for the organization’s care management operations and services, aligning with the organization’s mission, strategies, and objectives.
Position Description The ECM Program Manager leads and oversees care management operations and services, ensuring alignment with organizational goals and high-quality patient care. This role collaborates with clinical and operational leaders to design, implement, and evaluate care management programs, patient risk stratification, and care coordination across the continuum.
Responsibilities
Supervises the care management team.
Leads the implementation of care management programs designed to address patient needs, improve quality and services, and ensure appropriate utilization of services.
Implements plans and tools to meet organizational goals and objectives.
Collaborates with other departments to design, implement, and evaluate care management programs.
Leads complex projects focusing on performance improvement and transformation of care management programs for the served populations.
Supports development, implementation, and evaluation of pilots, programs and practices based on market-leading and evidence-based research and performance outcomes.
Ensures standardization and optimization of workflows of care models that are being spread and scaled.
Develops playbooks for care management models ready for spread and scale; works with clinical and operational leaders to perform patient risk stratification and outreach.
Organizes and facilitates meetings with key stakeholders involved in care coordination/case management programs.
Continuously evaluates market-leading and evidence-based research in care management.
Contributes to design, implementation, and evaluation of IT solutions to support care management documentation and monitoring of efforts.
Leads standardized deployment of an administrative case management tracking system/care management documentation.
Performs miscellaneous job-related duties as assigned.
Qualifications
Valid California Driver’s License.
Bachelor’s and/or Master’s degree in a health-related field is preferred.
3+ years of supervisor/leadership experience.
Experience in acute inpatient, rehabilitation, sub-acute, skilled facility, home care, ambulatory care management, or managed health plan.
Experience working in a multi-cultural setting; willingness to learn and understand diverse cultures and norms.
Experience in a community-based setting for 1–2 years preferred.
Basic computer skills; EMR experience preferred.
Strong communication skills and ability to convey medical information to professionals and the public.
Ability to work with confidentiality and handle multiple priorities with strong organization and time management.
Ability to develop and guide the team toward new approaches to improve processes and procedures.
