Job Title:
Social Worker (MSW)
Job Summary: Acts as an advocate to the residents and families of the MJH long term care nursing home area by delivering a broad spectrum of social work services in a qualitative, empathetic, and caring manner.
Responsibilities
Supports, counsels, and acts as an advocate for residents and family.
Completes all necessary and required social work documentation including psychosocial assessments, treatment plans, quarterly evaluations, MDS, supplemental notes, discharge plans, etc.
Coordinate and facilitate patient discharges by ordering all services needed post discharge from facility.
Interfaces with interdisciplinary staff members to share information on the resident.
Assists with discharge planning in a rehab setting.
Facilitates support groups.
Attends meetings as needed.
Job Requirements Education
Master’s degree in clinical social work required.
Experience
Three years of related experience.
Licenses/Certifications
N/A
Abilities Required
Excellent assessment and counseling skills.
Ability to work in a fast-paced social work position that involves discharge planning from a rehab setting.
Must be computer literate.
Excellent communication and interpersonal skills.
Knowledge of geriatrics/gerontology preferred.
Ability to work closely and well with other disciplines.
Thorough knowledge of psychosocial dynamics of individuals and families.
Functional Demands Environment /Work Conditions
Normal working condition with adequate lighting and ventilation.
Infectious Material Exposure
No routine exposure to blood/body fluids/tissues but may occasionally require performing unplanned activities which may involve exposure to blood/body fluids/tissues.
Organizational Expectations
Ensures that resident’s/patient’s rights are adhered to.
Demonstrates professionalism and accountability.
Demonstrates a caring attitude consistent with Miami Jewish EmpathicareSM toward MJH residents, patients, family members, employees, and other facility guests.

Ensures confidentiality and security of patients’ medical information.
Identifies and utilizes appropriate channels of communication.
Able to speak, read and write English.

Participates in departmental activities, meetings and in-services and follows established guidelines.
