The Department of Economic Security, Division of Developmental Disabilities (DDD), is seeking an experienced and highly motivated individual to join our team as a Case Management Consultant. DDD provides individuals with developmental disabilities, and their families, services and supports that are flexible, high quality, and member driven. These services enable individuals to exercise their rights and responsibilities of independent decision-making and engagement in the community. Applicants for this position cannot be licensed or certified by the DDD Office of Licensing Certification and Regulation, cannot have a principal interest in or have a spouse or close family with principal interest in a DDD Qualified Vendor Agreement, and cannot be employed or subcontracted by a DDD Qualified Vendor. Responsibilities
Provides technical assistance/consultative support to care coordination within the Division and with other systems of care by attending CFT/ART and other state-level meetings to coach support coordinators on the job and enhance DDD integrated care delivery. Participates in internal escalation to ensure member needs are met in a timely manner. Daily monitoring of utilization of services, including inpatient reports, emergency department reports, Behavioral Health Crisis contacts, newly determined SMI members, and special assistance. Committee member for Step down home, tracks members entrance and exit criteria, identifies treatment goals, tracks goals for completion, and assists in planning for successful discharge. Attends district huddles and prepares/presents trainings on BH Stigma Reduction, System of Care Principles, Habilitation, Early Childhood Autism Specialized (ECM) Requests, Crisis Planning, SMI Determination Process, Service Package Benefits Child/GMH/SMI Differences, and Discharge Planning from BH Inpatient Facility and BH Residential Facilities. Completes service approvals for Positive Behavior Assessment and Consultation Services (ASI), Habilitation Consultation (HAB-C), and second-level med reviews. Completes progress notes, reports, planning documents, and other correspondence related to the performance of job duties. Knowledge, Skills & Abilities (KSAs)
Knowledge in: Care and assistance of individuals with developmental disabilities and behavioral health needs. Principles and practices of support coordination, community resources, medical and behavioral health issues. Understands appropriate placement settings based on the assessed need of the member. Google Suites & Microsoft Office products (Word, Excel, Outlook, etc.). Skills in: Interpersonal relations as applied to contact with members, families, and representatives of governmental agencies, community organizations, and the public. Leadership skills to facilitate teachable moments for internal staff. Oral and written communication. Negotiation and conflict resolution. Work calmly and maintain effectiveness in situations of high stress. Ability to: Partner with stakeholders, Department of Child Safety (DCS), Arizona Health Care Cost Containment System (AHCCCS), Comprehensive Medical and Dental Plans (CMDP), Arizona Department of Corrections (ADOC), etc., for cross-system collaboration. Make prompt independent decisions based upon relevant facts and established processes. Handle sensitive information, maintain confidentiality. Work independently with minimal supervision. The ideal candidate for this position will have: A Bachelor’s degree in a human services or equivalent field from an accredited college or university and at least two years of professional experience with developmental disabilities; OR 4 years of professional case management/social work experience, including 2 years with developmental disabilities. • If this position requires driving or the use of a vehicle as an essential function of the job to conduct State business, then the following requirements apply: Driver’s License Requirements. Any employee operating a motor vehicle on State business must possess and retain a current, valid class-appropriate driver's license, complete all required training, and successfully pass all necessary driver's license record checks. The license must be current, unexpired, and neither revoked nor suspended. Employees who drive on state business are subject to Driver's license record checks, must maintain acceptable driving records, and must complete all required driver training. An employee who operated a personally owned vehicle on state business shall maintain the statutorily required liability insurance. • Employee shall have a valid Level One fingerprint clearance card issued pursuant to Arizona Revised Statutes 41-1968 in order to work with children and vulnerable adults. • Candidates for this position shall be subject to a search of the Department of Child Safety Central Registry and the Adult Protective Services Registry. • Successfully pass background and reference check; employment is contingent upon completion of the above-mentioned process and the Agency's ability to reasonably accommodate any restrictions. • All newly hired State employees are subject to and must successfully complete the Electronic Employment Eligibility Verification Program (E-Verify).
