The Outreach and Enrollment Counselor will conduct outreach and in-reach, educate, and assist clients enrolling in affordable health insurance coverage through Covered CA, Medi-Cal, Medicare, or the Children’s Health Insurance Program as well as non-health insurance programs such as Cal Fresh. Responsibilities Conducts outreach activities and presentations at school and community events such as health fairs, churches, farmers markets, grocery stores, and local businesses; events may be scheduled during weekday, evening or on weekends Networks and establishes community relationships and partnerships Understands and complies with monthly and yearly end goals and objectives in the various grant contracts and maintains complete data for monthly reporting Supports NHC efforts for patient retention as well as increasing new patient growth Presents health concepts to individuals and small groups Disseminates information about affordable health insurance coverage to families and individuals Conduct in-reach phone calls to current patients to offer enrollment assistance Provides enrollment assistance in affordable health insurance coverage through Covered CA, Medi-Cal and the Children’s Health Insurance Program as appropriate Provides education and enrollment assistance for the CalFresh program Provides education on other community resources and assistance programs, such as SDG&E CARE program Assists the applicant with questions regarding health insurance, healthcare access and other resources and ensures enrollment process is completed Refers applicants to their medical home and other needed resources at time of application completion Receives referrals from collaborating and partner agencies Performs inter-departmental referrals for patient in need of medical, dental, and behavioral health services or other available services Provides coverage updates to clients regarding their affordable health insurance Attends meetings and trainings necessary for the position sustainment and growth Prepare monthly quantitative and qualitative report on outreach/in-reach and application assistance activities Shares feedback from patient experiences to improve access to health care Keeps self and others informed of organizational activities and promotes mission and goals Conducts patient referrals to assigned Medicare agents as required Assists in the efforts to retain patients and support efforts at eliminating patient attrition Qualifications Education/Experience High school/GED required Covered California certification required within 90 days of employment One year of experience in government or community program eligibility/screening for Medi-Cal or Medi-Cal for families preferred Certified Enrollment Counselor for Covered CA experience preferred Additional Qualifications (Knowledge, Skills and Abilities) Excellent verbal and written communication skills, including superior composition, typing, and proofreading skills Ability to interpret a variety of instructions in written, oral, diagram, or schedule form Knowledgeable about and experience with health care and social service resources in the community preferred Ability to successfully manage multiple tasks simultaneously Excellent planning and organizational ability Ability to work as part of a team as well as independently Ability to work with highly confidential information in a professional and ethical manner Neighborhood Healthcare offers a generous benefit plan that includes: Partially company paid Medical, Dental, and Vision Plans.
