Overview
The County of Marin’s Department of Health & Human Services seeks a Social Service Worker II to provide integrated behavioral health, substance use, and housing services across multiple divisions. The role is collaborative, client‑centered, and travel‑based throughout Marin County. Responsibilities
Conduct comprehensive assessments, develop individualized service plans, and provide ongoing case management, including housing navigation, community resource coordination, and wellness promotion. Provide recovery‑focused substance use screening, assessment, counseling, and case management services to IMPACT team clients; collaborate with all team members, clients’ natural supports, and community partners. Manage a caseload of adults with serious mental illness, substance use disorder, homelessness, or transition from incarceration; conduct home visits, maintain accurate documentation, and monitor case outcomes. Coordinate care across systems (e.g., MediCal, CalFresh, CalWorks, public housing, justice system, and other county and external partners); maintain care coordination records and share information with health plans and other agencies. Serve on task forces, participate in community outreach activities, and mentor junior staff; promote culturally responsive, trauma‑informed practice and community engagement. Qualifications
Either possession of a Master’s Degree in Social Work or a two‑year counseling program by the time of appointment, or a Bachelor’s Degree with four years of adult social casework experience (two years at a level comparable to Social Service Worker I). Minimum of two years of experience in adult social services, behavioral health, or housing services at a level comparable to Social Service Worker I or I‑Bilingual. Strong knowledge of social work principles, evidence‑based practice, motivational interviewing, and case management; experience with crisis intervention and dual diagnosis populations. Proficiency in client assessment, intervention, case planning, and documentation; demonstrated ability to work independently with minimal supervision. Background check, fingerprinting, and LiveScan eligibility required. Must be able to travel throughout Marin County and maintain flexible schedule for on‑site and travel‑based duties. Working Conditions
Hybrid schedule—primarily remote with onsite county facility requirements for meetings, assignments, and disaster service duties. Regular on‑site work during initial probation period before establishing a hybrid schedule. Equal Employment Opportunity
