About The Role Chateau Rehab and Nursing Center seeks an experienced and dynamic
Co-Admission Director
to join our leadership team. The ideal candidate is organized, proactive, and passionate about helping families navigate the admissions process with compassion and efficiency.
Responsibilities
Monitor and manage all referrals with a 90%+ response rate within 15-30 minutes.
Identify and address operational barriers and assist leadership in developing effective solutions.
Complete accurate daily AM & PM census reports and distribute to designated parties.
Maintain accurate census data in Future Care PTS and CensusPro systems.
Collaborate with billing and interdisciplinary teams to ensure seamless admissions processes.
Communicate with marketing and outreach teams to maximize admissions opportunities.
Conduct facility tours, present admission packets, and explain facility services, policies, and resident rights.
Obtain necessary financial and insurance documentation to ensure accurate verification prior to admission.
Assist in daily Morning Report, Utilization Review, and Quality Assurance meetings.
Prepare and report monthly admissions statistics to leadership teams.
Requirements
Minimum 1 year of admissions experience in a skilled nursing facility or medical setting.
Proficient in Microsoft Office, Allscripts, Navihealth, EPIC, and PCC/Sigma Care.
Strong understanding of Medicare/Medicaid, PDPM, HMO rules, and CMS guidelines.
Excellent organizational, communication, and customer service skills.
Ability to work efficiently in a fast-paced environment while maintaining accuracy.
Proven leadership skills with the ability to coach and support team members.
Professional demeanor and adherence to facility dress code.
Must participate in all required facility communications, including WhatsApp group chats.
Enthusiastic, detail-oriented, and motivated attitude with the ability to manage multiple priorities.
