Project Coordinator Join to apply for the Project Coordinator role at City of Chicago.

Department: Department of Family and Support Services. Division: Senior Services Division. Number of positions: 2. Starting salary: $73,140.

Location: 1615 W Chicago Ave, Chicago, IL 60622. Work schedule: Monday thru Friday 9:00 A.M. to 5:00 P.M.; weekend/evenings as needed.

This position is open to the public and all current City employees covered under the City’s collective bargaining agreement with the American Federation of State, County, and Municipal Employees (AFSCME) – bargaining unit 1, 3, 4 & 5. Only employees in City job titles in this bargaining unit are eligible to bid.

Responsibilities

This position will support Senior Services including, but not limited to the Caregiver Portfolio, Dementia Initiatives, and other programs.

Conduct and convene community outreach events with DFSS staff, DFSS delegate agencies, and other community partners such as community resource fairs, presentations, trainings, and workshops on caregiving and dementia as well as coordination of outreach for Memory Cafes run by delegate agencies.

Establish awareness of Senior Services including but not limited to Caregiver Portfolio, Caregiver Education, dementia care services, and disseminating caregiver information and maintain rapport between community institutions such as Regional and Satellite Senior Centers, libraries, aldermanic offices, hospitals and DFSS.

Attend community meetings and serve as a liaison with City departments, public and private agencies and business organizations to secure commitments in providing services to program participants, community groups, sharing program, Divisional, and Departmental information and other related activities.

Support Delegate Agencies with caregiver program implementation.

Serve in special committees related to areas of concern of and protection for caregivers, older adults and individuals with disabilities.

Develop, oversee marketing strategies to encourage participation in related projects and programs and ensure distribution to relevant community outlets.

Communicate with caregivers or older adults and / or professionals who may be seeking resources.

Contact existing and potential participants for recruitment, retention, and follow up.

Refer clients to information and assistance team to link to other services. Project coordinator(s) will complete data collection and entry on Salesforce/Chi311 App, AgingIS, and TCARE and prepare periodic reports on activities, status, and progress of funder metrics and outputs.

Administer screeners, assessments and monitor data.

Input, approve Delegate Agency vouchers for payment and enter APS and EA entry and tracking.

Provide SHIP information and Medicare / Medicaid counseling.

Need to successfully complete training and certification for IDOA programs.

They will perform Divisional and other related duties as assigned.

Qualifications Graduation from an accredited college or university with a Bachelor's degree in Public or Business Administration, Social Sciences, Planning or a related field, supplemented by three years of progressively responsible experience in program administration, or an equivalent combination of training and experience is required.

Previous project management experience.

Previous work experience managing social service programs with an emphasis on Senior Services or the aging population.

Previous experience developing, managing and conducting community outreach plans.

Bilingual – English and Spanish.
